Phenylalkylamines

• Bradycardia or atrioventricular conduction delay.

• Constipation as the muscles of the gut wall do not contract as well.

• Drug interactions with autoimmune drugs.

Benzothiazepines

• Bradycardia and atrioventricular conduction effects.

• Occasional skin rash.

• Drug interactions with autoimmune drugs.

• Angiotensin-Converting Enzyme Inhibitors

Angiotensin-converting enzyme (ACE) inhibitors inhibit the activity of angiotensin-converting enzyme (Figure 26.6). Angiotensin makes the kidneys retain salt and water. ACE inhibitors:

• Act directly on the kidneys to cause salt and water retention.

• Cause the adrenal glands to reduce secretion of aldosterone: angiotensin is an aldosterone agonist (see Chapter 19 ‘Pharmacodynamics: how drugs elicit a physiological effect’, p. 140). Aldosterone increases salt and water reabsorption by the kidney tubules. This action is blocked by ACE inhibitors, leading to loss of salt and water.
